What is the Chronic Disease Management Plan (CDM)/Enhanced Primary Care (EPC) Plan?

The Chronic Disease Management (CDM) Plan formally known as the Enhanced Primary Care (EPC) Plan allows for Medicare rebates for allied health services for individuals with a chronic medical condition. If you have a chronic condition that your GP is managing under a Chronic Disease Management Plan, you may be eligible to claim a rebate through Medicare for these services. A chronic medical condition is defined as one that has persisted or is expected to persist for at least six months.

The CDM/EPC offers a maximum of five total rebates per calendar year for all allied health services. This means that if you are utilising multiple allied health services, you are entitled to a combined total of five rebates across all services, rather than five rebates for each individual service.

How do I access the CDM plan?

Your GP will determine your eligibility for these services and will provide a referral if necessary. For more information, please reach out to your GP or visit their website.

I have been referred to you on an CDM/EPC plan, how does it work?

You will need to provide us with your referral form. Once you submit the form, you will be eligible to claim up to five rebated sessions per calendar year, as long as your GP has registered your plan with Medicare.

Therapy sessions at Therability Speech Pathology must be paid in full upfront. As of 2024, the Medicare rebate is $61.80 per service. An invoice will be provided to you and you will be able to claim via Medicare online or in person.

You will need to present your Medicare card and an EFTPOS card so we can process an on-the-spot rebate for you.
